Parents: Indiana has new immunization requirements that affect almost every FT student, except those in grades 2-5, for the 2010-11 school year. The areas in bold print are new requirements for the grade levels listed and are reflective of recommendations by the CDC (Centers for Disease Control). Please send the updated record to your student’s school clinic when immunizations have been administered.

The new requirements are in effect for all current FT students as well. Please contact your healthcare provider soon to ensure that your student is fully immunized by August 2010. An appointment card must be on file if the provider is not able to vaccinate or schedule your student prior to the start of school 2010. New students may enroll but not attend until immunization records are up-to-date. For more information, please download the 2010-2011 immunization requirements below.

Another change will occur in RN reporting of immunizations, beginning next school year. Children & Hoosiers Immunization Registry Program (CHIRP) is a secure, confidential online system for storing and updating immunization records of children and adults in the state of Indiana. The state has mandated that every school nurse in Indiana provide district immunization status in this fashion, rather than the paper reports done in past years. This is for reporting purposes only and does NOT affect the FT procedures for gathering complete shot records. Your healthcare provider may already participate in this registry. This is a means for all immunization information to be in one place, even if you have changed providers often.

A copy of the permission form is available here. If you do NOT want your student’s records to be entered into the system, there is NO need for further action. If you would like to participate, please print a copy and either fax or send to your child’s school.
