Grade 6 – Additional Shot Requirements
All students in grade 6 for the 2011-2012 school year must have additional shots at the time they turn 11 years of age. Parents–please contact your student’s physician now to schedule an appointment when your student turns age 11. The shots needed are Tdap, MCV4 (meningococcal), and 2nd varicella (chickenpox) shots.
Students must have the shots OR a scheduled appointment on file to attend school on the first day in the fall, per IN law. If your student will not be eleven years old until later in the semester, please obtain an appointment and send a copy of the date to the EI clinic. Clinic staff will be contacting parents about appointment dates.
