The IN State Dept. of Health has changed immunization requirements for ALL students entering grades 6-12 next school year. This affects ALL FTMS-East students!
Parents, please contact your student’s physician now to determine if your student has received Tdap, MCV4 (meningococcal), and 2nd varicella shots. Please schedule an appointment if shots are needed.
Parents must provide an updated shot record from a physician at registration.
The Marion County Health Dept. has walk-in hours available throughout the week. Call 317-221-5700 for hours and directions. Students must have the shots or a scheduled appointment to attend in the fall per Indiana law. Parents cannot be contacted individually.
